CMS HETS Attestation: Submission Guide
The Centers for Medicare & Medicaid Services (CMS) requires healthcare providers (HCPs) to attest to their relationships with medical benefit eligibility vendors who access the HIPAA Eligibility Transaction System (HETS).
This attestation helps prevent unauthorized access to the Medicare HETS, ensuring that patients’ health information (PHI) remains secure and compliant.
What You Need to Do
All HCPs with an individual NPI must complete the HETS attestation for both of AssistRx’s eligibility vendors — Experian and Availity — by Spring 2026.
Failure to complete this process by the deadline could result in:
- Delays in AssistRx enrollment services and other support for your Medicare patients.
- Possible interruptions in therapy access for your Medicare patients.
When you enroll, you’ll need:
- The name of the person authorized to sign on your organization’s behalf
- Your email address (and optional alternate email address)
- Your Provider Transaction Access Number (PTAN)
- Your NPI
- Effective date of your relationship with your vendor or clearinghouse
- Termination date (if any) of your relationship with your vendor or clearinghouse
How to Complete the Attestation
Find your Medicare Administrative Contractor (MAC) region:

- Note: Your MAC may have several links covering different Medicare services (Part A, Part B, Home Health) — please complete an attestation for each.
- Complete a HETS EDI Enrollment form for each vendor using their unique IDs (UID):
- Experian UID: 6EQK
- Availity UID: QLUU

How Do Vendor UIDs Impact my AssistRx Attestation Requirements?
UIDs are tied solely to eligibility vendors. Completing the HETS attestation for Experian and/or Availity using their respective UIDs will satisfy AssistRx requirements. No separate AssistRx attestation is needed.
