CMS HETS Attestation: Submission Guide

The Centers for Medicare & Medicaid Services (CMS) requires healthcare providers (HCPs) to attest to their relationships with medical benefit eligibility vendors who access the HIPAA Eligibility Transaction System (HETS).

This attestation helps prevent unauthorized access to the Medicare HETS, ensuring that patients’ health information (PHI) remains secure and compliant.

What You Need to Do

All HCPs with individual NPI numbers must complete the HETS attestation language by May 11, 2026 to continue using electronic eligibility vendors. This includes the vendor utilized in your practice management system for eligibility and AssistRx’s two vendors—Experian and Availity.

Failure to complete this process by the deadline could result in:

  • Delays in AssistRx enrollment services and other support for your Medicare patients.
  • Possible interruptions in therapy access for your Medicare patients.
  • Interruptions for your Practice Management System’s eligibility services. Please check with your billing department to complete this attestation for your eligibility vendor if it is different from Experian or Availity.

When you enroll, you’ll need:

  • The name of the person authorized to sign on your organization’s behalf
  • Your email address (and optional alternate email address)
  • Your provider(s) and practice NPI(s)
  • Your provider(s) PTAN and practice PTAN(s)
  • Termination date (if any) of your relationship with your vendor or clearinghouse

How to Complete the Attestation

Find your Medicare Administrative Contractor (MAC) region:

  1. View the regional map below, and click on your region to see the applicable links. Each link will open in a new tab.
  2. Note: your MAC may have several links covering different Medicare services—Part A, Part B, Home Health and Hospice, etc. Please complete the attestation(s) for which you bill under.
  3. Complete a HETS EDI Enrollment form for each vendor using their unique IDs (UID):
